Questions about LDS verification

1. Since very few of the people I see are verified, when was this feature added?
2. After submitting the form, SS asked me to fill it out again rather than recognizing I had already sent it in once. Can you change it so that the "Get verified now" link goes away while status is pending at least?
3. Will there be an explanation of what your verification process is, such as looking up the local unit leader (bishop/president) and sending a copy of the picture for them to confirm as a member of the ward/branch?
